Have shortness of breath with exertion

Use Oxygen

Have had multiple hospitalizations/ER visits recently

Have difficulty with activities of daily living

Cognitive or behavioral impairment

If paying directly, we require payment at the time of the visit. All visits with our physician, when paid directly, cost $97.00, plus a processing fee of 3.5%.

How Quickly Can I Be Seen?
We make every effort to schedule all patients as quickly as possible. Unfortunately, there are times you cannot be seen on the dates you requested - we will work with you to make the visit as accommodating and convenient as possible. Typically, from the time you request an appointment with us by phone, email or web request, you can be scheduled to be seen within.

When Can I Be Seen?
For your safety and the safety of our providers, your visits will be scheduled between 9:00AM and 7:00PM. We do offer weekend and holiday appointments on a limited, case-by-case basis as determined by the availability of physician staff.
